Your Initial Appointment
To see one of our doctors you will need a valid referral from your GP. You will be asked to fill out our Patient Registration form prior to your appointment. This can be done online here or alternatively you can print out the Patient Registration Form here, complete the form and bring it along with you on the day of your appointment.
Please bring a copy of any investigations, reports, scans or test results concerning your condition.
Please register your arrival at the Reception desk before making your way to the waiting room prior to your appointment.
You will be charged a fee to see one of our specialists, a component of which will be rebated through Medicare. Please bring your Medicare card and details of your private health insurance.
If you are being booked for a procedure, you will be provided with full informed financial consent before you proceed.
Administration & Nursing Team
Our practice coordinator, Miss Georgia Brennan, is available to assist with any administrative task. Our practice nurse, Miss Grace Boag RN (Div 1), can be contacted for any clinical patient concerns.

Hours
Drummond House Ballarat is open 9am – 5pm Monday to Friday. Patients can also access 24/7 emergency care via the nearby Emergency Departments at St John of God Ballarat or Ballarat Health Services.
Parking
Parking is available onsite at Drummond House Ballarat, 414 Drummond Street North, Ballarat. There is disabled parking and access at the rear of the property.
